1. Applicant must be a 3 sport athlete having participated in an OHSAA recognized sport all 4 years of high school. Applicant must play a sport in the Fall,Winter and Spring seasons. 

2. Applicant must be active in community service. 

3. Applicant must complete an application and write an essay on how sports has impacted your life (500 words maximum- please attach). 

4. Applicant must submit two letters of recommendation. (These may not be from a family member). 

5. Applicant must be accepted to an institute of higher learning (i.e. university/college, junior college, trade school, technical school, etc.).
